For students to be eligible to participate in any extra-curricular activity they must maintain a grade of 70 or above in all subject areas on their report card and/or mid-quarter report.
In accordance with Glenburn Policy JJID, all students trying out and participating in soccer and cross country must also provide evidence of a physical examination by presenting a completed Glenburn School Sports Application prior to the start of any tryout sessions, practice, competition, or use of equipment. All students must also provide proof of medical insurance. If your child is in need of medical coverage, there is a school medical plan available for $10.00. A Glenburn School Sports Health History Consent Form and a Glenburn School Concussion Information Sheet must also be filled out every school year. These forms can be found on the school website http://10.0.3.37/athletic-forms (please print and fill out) or at the school in Mrs. Springer’s office.
We look forward to another exciting and enjoyable winter sports season. If you have any questions about whether your child is eligible or if you need the forms, just call the school at 947-8769.
